Items marked with an asterisk (*) are required fields that must be marked in order to submit the form. This information will be kept confidential and will be submitted directly to our financial secretary, Robyn Feeley. You may submit a paper commitment card if you prefer to do so. Paper Financial Commitment Cards will be located at the church Welcome Center desk and on the tables as you enter or exit the sanctuary, and can be returned to slotted drawer in the Welcome Center credenza or by mail.

Plan A: Combination Plan. (Under this plan 2.5% of your gifts to FBC's budget will be divided between the CBF and TBMB, with 79% going to CBF and 21% going to TBMB.)

Plan B: Cooperative Baptist Fellowship Giving Plan. (Under this plan 2.5% your gifts to FBC's budget will be allocated to the Cooperative Baptist Fellowship and Tennessee Cooperative Baptist Fellowship)

Plan C: Tennessee Baptist Mission Board (Under this plan 2.5% of your gifts to FBC's budget will be allocated to the TBMB. These funds will be directed to Tennessee Baptist Disaster Relief.)
